Risk, Need, and Payments in Health Systems: International Experiences in Risk Adjustment
Health policymakers are increasingly confronted with constrained public finances and competing priorities. Future-focused health systems must therefore rethink how resources are allocated and how funding mechanisms can better reflect population health needs.

This talk explores how healthcare resources are allocated across different health system frameworks around the world. It focuses on risk adjustment, a mechanism used to align payments with the expected health needs of populations by incorporating factors such as demographics and health status to predict healthcare costs.

Countries apply risk adjustment in different ways. In systems such as the Netherlands, Germany, and the United States, it is used to redistribute funds among competing health insurers. In other contexts, including Italy, England’s NHS, or Australia’s public health system, similar approaches are used to allocate budgets to providers, hospitals, or regional health authorities.

The session compares these approaches and discusses how institutional design shapes the role of risk adjustment in promoting equity, efficiency, and financial sustainability in health system financing.

Dr Francesco Paolucci | Professor
Newcastle Business School,  University of Newcastle in Australia | School of Economics, Management & Statistics, University of Bologna, Italy

Dr Francesco Paolucci is Professor in Health Economics in the Newcastle Business School at The University of Newcastle in Australia, and at the School of Economics, Management & Statistics, University of Bologna in Italy.  A scholar and advisor, over the last decade, he has published extensively, including one single-authored book and three edited special issues in peer-reviewed journals, as well as over 100 academic peer-reviewed articles, book chapters and book reviews in the areas of public policy, economics and management with a particular focus on healthcare nationally and internationally. In 2016 he received a ‘40under40’ Business News award in WA for his entrepreneurial and business leadership. Prof Paolucci is a permanent member of the Risk Adjustment Network, which is the leading network in health insurance and financing research and advisory globally, and the country representative for Australia.
